Step 1: Download and complete the Esquao Awards nomination form.

Step 2: Gather the following:

• Three letters of support.
• A high resolution photo of the nominee.
• Description of the nominee (Max 250 words).

Step 3: Send all of the documents (a total of four separate documents) to esquao@iaaw.ca
